Please be sure to fully submit the intake form and only complete it ONE time. You will see the “Thank You” page when it has been fully submitted.
These forms are not the waitlist, be sure to go to the scheduling page to book your first appointment or to join the waitlist there. Please be as detailed as possible on best dates and times, and dates that will not work. I will be in touch via text if an appointment time opens up.
