Please be sure to fully submit the intake form and only complete it only ONE time. You will see the “Thank You” page when it has been fully submitted. (If you receive an email to complete your forms you will find the link to those specific forms in your confirmation email, please complete through that booking link as those are separate forms).
These forms are not the waitlist, be sure to go to the scheduling page to book your first appointment or to join the waitlist there. Please be as detailed as possible on best dates and times, and dates that will not work. I will be in touch via text if an appointment time opens up.
